Description: The Medical College of Wisconsin\'s Department of
Psychiatry and Mental Behavior Medicine is seeking a Nurse Practitioner
or Physician Assistant to provide primary or specialty medical care
within defined scope of practice under the supervision of a licensed
physician and in accordance with MCW/FMLH/CHW policies and procedures
located in Menomonee Falls, WI at North Hills Health Center. The
Advanced Practice Provider (APP) works as a member of a
multidisciplinary health care team who will contribute to excellence in
program administration, patient care, patient safety, research, quality
improvement, and teaching. The Ideal Candidate will be a clinically
experienced APP with excellent communication skills, objective decision
making, and collegiality, wanting to be part of a thriving and very
collaborative academic practice at the Medical College of Wisconsin
(MCW). Areas of responsibility will include, but not be limited to
providing direct patient care to an adult patient population in an
outpatient setting. In this role you will, Collaborates with
interdisciplinary health care teams in diagnosis and treatment of
conditions requiring Psychiatry and Behavioral Health intervention in
outpatient environment. Writes accurate and timely progress notes,
procedure notes, and/or admission and transfer orders. Facilitates and
communicates with patient, family, and staff to promote continuity of
care across the continuum of care. Core Advanced Practice
Responsibilities Perform comprehensive histories and physical exams,
identifies normal and abnormal findings, including physical, cognitive,
and psychosocial status. Order, perform, and interpret screening and
diagnostic procedures and tests, and appropriately communicates and
documents information Write accurate and timely progress notes,
procedure notes, and/or admission and transfer orders Develop
pharmaceutical and non-pharmaceutical treatment plans. Monitor
effectiveness of plan of care Educate patients on disease and treatment
plans. Perform procedures as trained and appropriately credentialed.
Document within the Electronic Medical Record (EMR) and completes other
necessary documents. Collaborate and/or consults with physicians and
other health care providers as appropriate. Educates, counsels, and
assists patients and families regarding current health problems, health
maintenance activities and preventive behaviors. Attend required
