GENERAL RESPONSIBILITIES:

The Integrated Behavioral Health Clinician at Whitney Young Health (WYH) participates as part of a multidisciplinary team in an integrated behavioral healthcare setting providing a recovery oriented system of care to persons with substance use, mental health disorders and chronic co-morbid health issues. This position is held by a Licensed Clinical Social Worker (LCSW) and plays an integral role in integrating services to successfully ensure that individuals across the health-care domains receive all services necessary to experience optimal physical and mental health. The Integrated Behavioral Health Clinician (LCSW) conducts screenings and assessments, provides evidence-based individual, and family and group therapy and individualized treatment planning that addresses integrated health care needs. The LCSW in this role is responsible to ensure that there is swift access to care, that performance outcomes are met and; patients experience satisfaction with services (Triple AIM). The Integrated Behavioral Health Clinician recognizes and respects the inherent rights of privacy, confidentiality, dignity and respect of all persons. Coordinates and consults with other staff and service providers to support the best the attainment of treatment goals and positive outcomes.

SPECIFIC RESPONSIBILITIES:

  • Conducts face-to-face comprehensive biopsychosocial assessments and mental status exams, as well as a diagnostic assessment for all clients initiating care, and reassesses patients on an ongoing basis.

  • Develops a comprehensive, individualized treatment plan in collaboration with the multidisciplinary team and client. Oversees the periodic re-evaluation and revision of the treatment plan as necessary over the life of the client’s care.

  • Exhibits knowledge and skill in all functions of the electronic medical record pertinent to successful job performance.

  • Provides evidenced based individual, group, and family counseling as assigned.

  • Provides crisis intervention services as indicated.

  • Provides case management services as necessary and ensures coordination of care amongst service providers.

  • Maintains and protects accurate and timely documentation in case records according to agency policy and regulatory requirements.

  • Contributes to the provision of a safe and respectful environment for treatment.

  • Facilitates the collaboration between primary care and behavioral health care that includes participating in multidisciplinary collaboration toward the achievement of behavioral and physical health care outcomes.

  • Develops and networks with other community service providers to provide the full continuum of care needs identified by the multidisciplinary treatment team and supervisor.

  • Makes and completes all referrals to other community service providers as appropriate.

  • Participates in professional growth activities including ongoing education/training.

  • Maintains a professional relationship with staff and other providers and the community.

  • Refers clients to community social support services.

  • Provides staff trainings, as assigned.

  • Participates in individual and group supervision and; reports all pertinent information to supervisor.

  • Participates in staff meetings and other meetings, as assigned.

  • Utilizes the DSM V Diagnostic and Statistical Manual in the diagnostic assessment of all clients.

  • Maintains a caseload as assigned.

  • Completes Continuing Education requirements per licensing requirements.

  • Abides by all professional licensure Canon of ethics and insures that license to practice is maintained.

  • Demonstrates excellence in both internal and external customer service.

  • Understands and is able to effectively communicate HIPAA compliance, corporate compliance and client confidentiality.

  • Ensures and/or remains in compliance with local, state, and federal regulation, i.e. OASAS, NYS DOH.

  • Adheres to the National Patient Safety Goals as defined by the Joint Commission on Accreditation of Healthcare Organizations and the Whitney M. Young Jr. Health Services.

  • Demonstrates excellence in both internal and external customer service.

  • Completes other duties as assigned.

Requirements

MINIMUM QUALIFICATIONS:

Master’s Degree in Social Work with a LCSW from New York State; three to five years’ work experience providing behavioral health treatment services preferably in a medical setting; Strong individual, and group therapy skills with clients who have substance use, mental health disorders and co-morbid disorders. Strong assessment and diagnostic skills; Knowledge of recovery concepts, self-help groups, and community resources; Provides excellent customer service and demonstrates a high level of cultural competency; Ability to adhere to strict confidentiality guidelines; Working knowledge of computers and electronic health records.

PREFERRED QUALIFICATIONS:

Experience working in a community health care setting.

Salary range: $68,000 - $75,000 annually
