At Whitney Young Health, Health Home Care Coordination and service providers work together to make sure children get the care and services they need to stay healthy at the right time in the right amounts. A dedicated Health Home Care Coordinator is assigned to a child and is responsible for coordinating all of their health services. Care Coordinators help the child and family understand their chronic conditions, manage their symptoms and ensure that all treatment team partners involved with the child are working together.

Whitney Young Health (WYH), in partnership with Capital Region Health Connections, is committed to providing comprehensive community-based case management services to individuals with chronic medical conditions and co-occurring mental health and substance abuse disorders, through Health Home Care Coordination across Albany and Rensselaer counties.

Job Summary:

Using a patient-centered approach, Health Home Care Coordinators assist individuals enrolled in the program manage their conditions to achieve optimal health outcomes by developing individualized care plans designed to link individuals to necessary services; such as, housing, health care services, mental health and substance use providers, social services (food, entitlement benefits, transportation), community programs, and more.

The ideal candidate for this position possesses excellent written and verbal communication skills, knowledge of community resources, and strong time management/organizational skills. The person that assumes this position demonstrates computer proficiency, good judgement, establishes healthy boundaries, has the ability to work independently, and adheres to strict confidentiality guidelines.

Requirements

Minimum Qualifications

  • Associate’s degree in human services or a related field, and previous experience working with people with behavioral health disorders, HIV/AIDS, children, & families, ID/DD, or substance use disorders

  • Valid NYS Driver License in good standing (verified annually), and proof of auto insurance in compliance with NYS mandatory limits and coverage

Preferred Qualifications

  • Bachelor’s degree in a health or human services related field such as psychology, sociology, social work, or nursing, with at least one year of related Health Home Care Coordination work experience in a medical setting.

  • Bilingual (English/Spanish)

Salary range: $19.00 - $21.50 hourly
