Insight Global Care Transition Coordinator in Alexandria, Louisiana

Job Description

One of Insight Global's largest home health and hospice clients is looking for a Care Transition Coordinator to join their local agency in Alexandria, LA.

The Care Transition Coordinators primary responsibility is to facilitate a seamless transition for patients

discharging from a facility setting to the care of the local hospice agency.

The Care Transition Coordinator responsibilities are as follows:

  • Responsible for achievement of admission goals expectations as established at hire or at review of annual agency budget goals.

  • Assists the hospice agency with the preparation for accepting care of the patient post discharge from the hospital.

  • Assists the Administrator with execution of contracts for facility-based services for hospice patients.

  • Explains hospice services and agency procedures to the patient and their family members.

  • Involves the family and caregivers in the educational process and assesses post-discharge educational coaching needs.

Participate in bi-weekly IDG meetings, as necessary to give an update regarding accounts, customer needs, and progress towards agency growth strategies.

Required Skills & Experience:

  • Active RN license in the state of Louisiana

  • 2 Years of practical RN experience, preferably in hospice

The schedule for the incumbent will be Monday-Friday 8am-4:30pm-with one weekend a month of sales on call.

The salary range for this role will be anywhere between $80,000 - $85,000 based on relevant experience + a $2,5000 sign-on bonus and mileage reimbursement.

Skills and Requirements

  • Active RN license in the state of Louisiana

  • 2 Years of practical RN experience

  • Prior Hospice or Home Health experience - Hospice sales experience

  • Medical sales experience null
