St. Mary's Healthcare Pharmacy Technician Medication Coordinator in Amsterdam, New York

Job Requisition: REQ2720 Employment Type: Full-Time Shift: Rotating Hours Per Week: 40 Hours

Hiring Range : $17.98- $22.71

The actual compensation for this position will be determined based on experience and other factors permitted by law.

Responsibilities:

  • Recruits and enrolls patients utilizing enrollment forms and copy the patient’s insurance card.

  • Facilitates the collection of prescriptions to be sent to the pharmacy for dispensing and pick up the medication to be delivered to the patient’s bedside.

  • Ensures all consumer insurance information is up to date in the Pharmacy system and is properly charged for the medications dispensed.

  • Assists consumers in contacting the pharmacist regarding all their medication questions and ensure a prompt response to their questions.

  • Assists clinic staff and pharmacy with prior authorizations and Patient Assistance programs as needed.

  • Medication delivery when needed and appropriate.Ensures that medications are stored and transported properly (refrigerated drugs are kept in the refrigerator, pen-tips are not stored on insulin pens, etc.).

  • Obtains medication histories on all assigned patients during scheduled shifts.Performs patient and/or caregiver interviews to obtain dose and frequency of current medications including prescriptions, over-the-counter medications, and herbal supplements.

  • Calls outpatient pharmacies or other healthcare facilities to confirm medication regimens or investigate discrepancies when necessary.Communicates medication related issues to all appropriate heath care professionals.

  • Participate in ADM maintenance including, restocking, expiration date checking and general cleanliness. Will periodically review inventory stocked in the ADM's to ensure appropriate medications are present.Participates in continuous quality assessment and improvement activities and medication safety initiatives. Participates in or leads various committees, task forces, and performance improvement teams as assigned.

Education, Training & Experience

  • High school diploma/GED required. Associate or Bachelor’s degree preferred. Minimum of one (1) year health care experience required.

  • Strong working knowledge of brand and generic drug names. ·

  • Basic familiarity with computers. Ability to navigate at a basic level within web-based applications.

Certifications / Licensure:

  • Completion of technician training program and national Pharmacy Technician Certification required upon hire or within one (1) year of hire. Valid NYS Driver’s license required
