Job Summary
Interventional Radiology utilizes a large multidisciplinary team to provide clinical care to pre- and post-procedural patients, managing in- and out-patients with complex, acute, critical and chronic health conditions. The IR Clinical Care Coordinator will provide support to the APP team, other members of the healthcare team, referring providers, patients and families. The IR Clinical Care Coordinator must demonstrate exceptional organizational, prioritization and communication skills, be able to work effectively with a multidisciplinary team, handling multiple responsibilities in a fast paced and dynamic environment. The successful candidate must demonstrate effective problem solving skills, excellent attendance, and proficient computer skills. The successful candidate must adhere to a high standard of personal and professional conduct; possess excellent customer service, interpersonal, and communication skills; and demonstrated ability to interact with a diverse population and staff at all levels.
The IR Clinical Care Coordinator will provide comprehensive professional nursing support for Interventional Radiology clinic and procedure patients, including: coordinating care and follow up for patients with complex acute and chronic disorders across the care continuum; performing telephone triage; providing expert nurse instruction and consultation to members of the health care team; identifying gaps and needs of the patient population and implementing interventions to improve patient outcomes. This individual must develop partnerships with other clinical programs to standardize patient care processes, facilitate and maintain quality improvement/LEAN activities in Interventional Radiology procedure planning and clinic; provide education and coordination for pre-procedure patients and families seen in clinic and as inpatients

Hourly range for RSAM Registered Nurses (12+ months experience) $41.44-$67.30
Evening Shift Differential-$3.00 / hour
Night Shift Differential- $4.00 / hour
Day Shift Weekend Differential- $2.90 / hour
Evening Shift Weekend Differential- $5.90 / hour
Night Shift Weekend Differential- $6.90 / hour
Charge Nurse Differential- $1.00 / hour

Responsibilities*
Primary responsibilities include, but are not limited to:
Triages and addresses phone calls including post-procedure questions/complications and pre-procedure questions
Reviews clinic referrals and coordinates clinic scheduling
Responds to emails and inbasket messages from physicians, clerks, nurses and referring providers, including new referrals
Review incoming requests for procedures, request outside records, images and labs and coordinate clinic consult in a timely manner.
Coordination of care for clinic and procedure patients.
Post procedure follow up with patients and to schedule follow up clinic appointments
Reviews upcoming procedures for appropriate scheduling and readiness (labs, orders, equipment, consents, imaging, etc). Attends weekly scheduling meeting.
Arranges for preauthorization for medications, complete and submit required insurance forms.
Arranges for prior authorization for requested imaging (MRI, CT, DVU).
Point of contact for clerical questions
Point of contact for patient questions, pre-procedure education
Obtains OSH imaging and uploads
Other duties as assigned

ENC- COMPETENT NE
Actual RSAM LEVEL and salary will be determined at time of hire.
RSAM levels range from ENC-COMPETENT-NE to ENC-EXPERT-NE TO ENC-MASTERY-NE.
Required qualifications must be met by the candidate in order to be interviewed and considered for the position.

Required Qualifications*
Current licensure as a Registered Nurse in the State of Michigan
Bachelor of Science Degree in Nursing OR Associates Degree/Diploma in Nursing along with a Master's Degree in Nursing.
Minimum 5 years of current RN experience providing care to adult procedural or surgical patients
2 years of recent (within the previous 5 years) experience in a clinical leadership role such as charge nurse, preceptor or similar role demonstrating organizational, prioritization and multi-tasking skills.
Previous work performance demonstrating self-direction, autonomy and flexibility.
Strong leadership skills as evidenced by previous evaluation documentation.
Previous work performance demonstrating excellent critical thinking and problem-solving skills
Demonstrated expert communication skills as evidenced by interviews, references, and performance evaluations.
Participation in unit activities that demonstrate the ability to work collaboratively with and positively contribute to a team approach to patient care.
Demonstrated high standards of personal and professional conduct and attendance as evidenced by interviews, references and performance evaluations.

Previous experience providing care to adult Interventional Radiology procedure patients and post-procedure management

Day shift: Monday - Friday; Varied hours
Location: University Hospital, Cardiovascular Center and IR clinics (including ACS clinics)
All new employees will be expected to float to a designated unit(s) in times of low census.
