Job Summary and Qualifications

The Lung Nodule Coordinator independently manages all aspects of the Lung Nodule Clinic related to patient navigation and lung clinic appointments, in conjuction with physician(s) and administrative leadership. The focus of this role will be to manage and follow up on lung nodules found through various avenues including but not limited to highrisk lung screenings, incidental findings in the Emergency Room and inpatient admissions. The Lung Nodule Coordinator will coordinate the next level of care for patients with lung nodules and assist patients, clinicians and program stakeholders in the collection, reporting and presentation of program outcomes

What you will do in this role:

  • Serve as patient advocate from first suspicious finding to diagnosis.

  • Independently manages all aspects of the Lung Nodule Clinic to include but not limited to: Patient identification and navigation, Medical Record review and physician collaboration

  • Streamline care coordination and paperwork by assisting patient(s) with scheduling appointment(s) and exam preparation.

  • Support providers and assist patients in understanding their treatment options and resources available (e.g. Smoking Cessation and Pulmonary Rehabilitation).

  • Empower patients to remove barriers with issues of insurance, transportation, child care, financial resources in conjunction with internal and external resources (ex. Social workers, Financial Counselors)

  • Conduct follow-up conversations as needed with all patients and communicate concerns, changes, or social needs in patient health to appropriate MD or other appropriate care providers.

What qualifications you will need:

  • Basic Cardiac Life Support must be obtained within 30 days of employment start date

  • Registered Nurse

  • Associate Degree, or Bachelors Degree
