Position Number: 2000-2403Position Location: This is a full-time position located in Augusta. This opportunity allows partial telework with management approval.

The Maine CDC Breast and Cervical Health Program is looking for a Patient Navigator to join our team and coordinate delivery of breast and cervical cancer screening and follow-up diagnostic services to underserved clients statewide. This work helps protect the health of eligible people by diagnosing breast/cervical cancer early when it is most treatable and helps reduce health inequities by improving access to treatment for eligible Maine people diagnosed with breast or cervical cancer.

The position coordinates patient navigation services for enrolled clients who need help scheduling breast/cervical screenings, who have abnormal breast/cervical cancer screening results, or who have a diagnosis of breast/cervical cancer. You will review incoming abnormal clinical data and ensure follow-up on abnormal screening results, working in conjunction with healthcare providers whenever necessary. As Patient Navigator, you will also ensure timely access to treatment services by determining eligibility for the MaineCare Treatment Act for breast/cervical cancer. You will make independent decisions on the appropriate information / assessment processes to follow and take actions in accordance with standard practices.

Core Responsibilities

As a key member of the Maine CDC Breast and Cervical Health Program (MBCHP) team, your core responsibilities will include:

• Assisting MBCHP clients with accessing resources, scheduling appointments, and/or receiving further diagnostic testing as needed;

• Enrolling eligible clients diagnosed with breast/cervical cancer onto the MaineCare Treatment Act and reviewing cases annually to ensure continued client eligibility;

• Conducting telephonic consultation with MBCHP enrolled clients and healthcare providers, fully documenting all communications in MBCHP’s Data Management and Reporting System phone-log module;

• Working to ensure program compliance via MBCHP protocols and procedures regarding breast/cervical cancer risk assessment of clients;

• Helping the MBCHP team maintain, analyze, and update policies, procedures, rules, and regulations; and

• Assisting with preparation of semi-annual reports submitted to federal CDC.

MINIMUM QUALIFICATIONS: (Entry level knowledges, skills, and/or abilities may be acquired through, BUT ARE NOT LIMITED TO the following coursework/training and/or experience).

Six (6) years of education and/or professional experience in social services casework, social services program development/delivery, or social services related fiscal/budget administration/analysis work.

Preferred candidates will also have:

o Excellent oral and written communication skills;

o Excellent listening skills;

o Strong teamwork and customer service skills;

o Prior experience working with patient data/medical terminology, in a medical office setting, or in a cancer-related setting;

o Computer skills, including knowledge of Microsoft Office Products and database management;

o Ability to prioritize assigned tasks to meet deadlines;

o Ability to maintain confidential client information in compliance with HIPAA (the Health Insurance Portability and Accountability Act);

o Ability to multi-task while maintaining extreme attention to detail on written communications;

o Ability to be flexible;

o Excellent phone etiquette skills.
