Responsible for assisting activities of adult day care programs and services. Collaborates with staff and volunteers in identifying the health, education, and psycho-social needs of individuals in the community and develops programs, projects and activities based on the assessed needs. Assures that all patient needs are met in terms of personal care and all other activities.

Principal Accountabilities:

1. Supports social activities to keep members engaged.

2. Provides interventions to assist Members behavior during these activities.

3. Leads activities at the direction of the Activity leader.

4. Provides ADL assistance, bathing dressing and grooming as needed for members.

5. Set up and breakdown programming rooms for activities

6. Assists with Meals times and serves refreshments in accordance with Member diet restrictions.

7. Performs other related duties as assigned.

   Qualifications for Internal Candidates

Required:

1. High School Diploma or equivalent.

2. CHHA or CNA certification

Preferred:

1. 1 year of experience in assisted living, skilled nursing, or Adult Day Care experience or related field.
