USA Job Details Description The ideal candidate will have leadership
experience as part of a clinical team management co-occuring mental
health and medical conditions. This position is based out of CHD\'s
specialty intensive medical group home in Belchertown, and provides
clinical oversight and planning services to that site as well as to
outreach clients, as needed. Responsibilities may include: reviews
Persons? treatment plans develops crisis intervention plans interacts
with Persons in order to assess needs, provide information, discuss
problems, and encourage growth and development provides informal
counseling monitors Persons? progress locates housing for Persons;
assists with physical transition accompanies Persons to medical review
or crisis evaluation intervenes in the event of a Person?s crisis,
suspected illegal activity, or any behavior consisting of an incident in
the program to resolve the situation and insure the safety of Persons
and others insures that Persons receive maximum entitlement benefits
adheres to program procedure manual develops and monitors Persons?
management systems provides clinical consultation to direct care staff
participates in program management meetings delivers services to any
Persons in program in the event that the assigned staff is unavailable
participates in and helps develop program trainings participates in
on-call system to ensure staff coverage and resolve crisis
Qualifications Skills Preferred Medication Admin, Current - Novice First
Aid Certif, Current - Novice CPR Certif, Current - Novice Education
Preferred Masters of Science or better. Licenses & Certifications
