Job Description:As a Case Manager with us, you will have the opportunity
to be involved in the coordination of care and services for patients admitted
to Intermountain Health Platte Valley Medical Center who are identified as
needing assistance or meeting Care Management criteria.
Scope
As a Social Work Case Manager, you need to know how to:
Coordinate the care and services for patients identified as needing assistance
or meeting care management criteria.
Assess the patient's prior level of functioning, access to and/or use of
community resources and available support systems.
Identify appropriate admission and continued stay issues.
Attend, facilitate, and participate in rounds and case conferences.
Identify needs, facilitate, or provide education to physicians and
ancillary departments/nursing units regarding case management, discharge
planning process, and roles.
Participate in multidisciplinary groups and development of guidelines.
Minimum Qualifications
Master's degree in social work from an accredited program is required
Current valid LCSW license or LSW is required
LCSW required within 3 years of employment is required
Preferred Qualifications
Care Management Certification is preferred
Physical Requirements:
Interact with others by effectively communicating, both orally and in
writing.- and -Operate computers and other office equipment requiring the
ability to move fingers and hands.- and -See and read computer monitors and
documents.- and -Remain sitting or standing for long periods of time to
perform work on a computer, telephone, or other equipment.
