Schedule: M - F 10 to 6 EST

Salary Range: $96K - $110K*

Older Adult ACT Team Leader (Program Director)

Position Overview:

The Assertive Community Treatment (ACT) Team Leader is responsible for the day-to-day oversight, coordination, and compliance of a team of professionals bringing a full range of psychiatric rehabilitation services to older adults aged 50+ years of age or older living in the community. Responsibilities include staff supervision, maintaining compliance with all relevant policies and regulations and fidelity to the ACT model, closely monitoring the program’s billing/utilization and revenue, and developing and maintaining relationships with the community and other service providers. The position requires community-based work, case coverage in times of shortage, and flexible hours.

Reports to: VP of Programs

Job Duties/Responsibilities:

  • Maintain a census of 48 consumers

  • Ensure ACT team staffing complies with OMH ACT Program Guidelines and/or other relevant regulations

  • Provide ongoing clinical and administrative supervision for ACT Team members

  • Ensure all ACT team staff complete all required training offered by external sources as well as training offered within the organization

  • Facilitate daily team meetings at least 4 times per week

  • Coordinate program coverage, ensuring adequate staffing and 24-hour staff availability 365 days/year

  • Ensure the use of evidence-based practices within the program as required by OMH ACT Program Guidelines and/or other regulations

  • Completion of CAIRS reports

  • Maintain and uphold policies and procedures reflective of OMH ACT Program Guidelines and any DOHMH expectations, including all required training

  • Closely monitor billing and revenue; ensure monthly billing is completed accurately and submitted in a timely fashion, and promptly resolve any challenges preventing the program from receiving reimbursements

  • Submit Quality Improvement (QI) reports, level of service reports, outcome data, incident reports, and other required reports in a timely fashion as required by funders or by internal Vibrant stakeholders

  • Ensure the program maintains satisfactory internal and external audit ratings; promptly address any challenges identified during internal/external audits

  • Complete monthly program compliance audits of all required program operations to ensure the team is operating in congruence with State, City, and Program regulations, policies, and procedures.

  • Coordination between the team and systems within New York City, including (but not limited to): AOT, Psychiatric Emergency Services, Housing, and Adult Protective Services

  • Maintain positive connections with the community, referral sources, and collateral service providers, including the establishment of Linkage Agreements

  • Work as part of the team in the community, meeting directly with consumers and providing services, as well as occasional provision of field supervision

  • Attend OMH ACT Team Leader meetings

  • Requires some evening/weekend hours and 24/7 on-call availability.

  • All other duties as assigned

Qualifications:

  • Master’s degree (license preferred) in social work, psychology, rehabilitation counseling, or related field required OR licensure/registration as a registered nurse, nurse practitioner, or physician

  • Candidates must be enrolled or eligible to enroll in NYS Medicaid Program within 30 days of hire (NYS: LCSW, Licensed Psychologist, Licensed Mental Health Practitioner, Licensed Physician or Licensed Nurse Practitioner)

  • At least 4 years of experience working with an SPMI population is required; experience working with older adults a strong plus

  • At least 2 years of supervisory experience required

  • Excellent organizational and communication skills, both written and verbal

  • Excellent computer literacy skills

  • Strong track record of providing effective supervision to clinical and non-clinical team members

  • Understanding of evidence-based practices

  • Bilingual/bicultural a strong plus

Physical and Sensory Requirements

  • A mix of sedentary work, using standard office equipment including a computer, keyboard, and telephone, and community work, including visits to schools, partner sites, outreach events

  • Ability to work collaboratively with staff, program participants, service providers, and other stakeholders

  • Ability to remain calm and composed under stress

  • Ability to respond to telephones and other auditory stimulation

  • Ability to evaluate/interpret information and make independent judgments/decisions
