PURPOSE OF POSITION
The Licensed Physical Therapy Assistant (LPTA) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Physical Therapist (PT).
RESPONSIBILITIES & EXPECTATIONS
Understands and adheres to established agency policies and procedures.
Provides physician prescribed physical therapy under a plan of care established by the PT
Improves or minimizes residual physical disabilities of the patient
Returns the individual to optimum and productive level within the patient's capabilities
Participates with all other health care personnel in patient care planning
Performs all skilled procedures as ordered by physician and according to the plan of care established by the PT
Consults with the PT regarding change in treatment
Instructs patients and family members in home programs and activities of daily living
Performs services planned, delegated and supervised by the PT
Documents accurate and complete notes of observations, treatments and care of patients in a timely manner as per agency policy
Documents the patient's response to the plan of care
Contributes to the evaluation of the patient's response to the plan of care through documentation and verbal communication with other members of the interdisciplinary team
Reports changes in the patient's condition to the physician and PT
Contributes to the revision of the plan of care
Demonstrates respect for personal privacy and property
Supports the patient's and family's diversity
Attends all case conferences as scheduled
Participates in case conference meetings to discuss the need for involvement of other members of the interdisciplinary team
Performs other activities or tasks within the scope of practice of the licensed physical therapy assistant as requested by the Director of Clinical Operations
Meets daily average productivity standard established by the agency
Completes assigned responsibilities in time frame required by agency policies
Displays a positive and professional image and attitude in all relationships with patients, families, peers and in the community
Participates in the Quality Assessment Performance Improvement (QAPI) program
Complies with agency infection control and safety policies
Completes annual competency requirements
Consistently communicates requests, suggestions, concerns or problems to the appropriate personnel
Demonstrates flexibility in meeting the needs of the program and responds positively to changes in workflow
Protects patient health information according to the HIPAA Privacy Rule and Aultman policies
Creates and fosters a work environment that is consistent with the Mission, Vision and Values of the Aultman Health Foundation
Supports strategic organizational goals
Abides by the Aultman Code of Conduct at all times

QUALIFICATIONS
Graduate from a two-year college level program approved by the American Physical Therapy Association
Currently licensed, registered or certified in the state of Ohio
Two (2) years' experience, preferred.
Ability to exercise initiative and independent judgment
Current BLS certification
Valid OH Driver's License
