SUMMARY
This person will follow the directives of theVP of Health Operationsin
all areas of care coordination. Responsibilities include but not limited
to: immediately responding to providers and support staff that have
screened and identified patients at risk of suicide during a medical
visit, making contact via video or phone to complete a full Columbia
suicide assessment, make recommendations, ensure client is not at risk
for immediate harm to self, assign to a therapy and/or psychiatric
provider and follow the patient through to their first visit with a
clinic based program.
This position is home based and requires high speed internet at the
home office that meets the technical specifications to ensure quality
interactions with patients.
In addition, this position can carry a caseload and will be providing
individual counseling sessions, documenting patient progress, and
offering consultation to medical providers as needed. This position will
have access to privileged information, and confidentially must be
maintained at all times.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Be available during scheduled hours to immediately respond to
providers and support staff that have screened and identified
patients at risk of suicide during a medical visit.
- Making contact via phone or in person and complete a full suicide
assessment
- Develop safety plans or other actions to ensure client is not at
risk for immediate harm to self.
- Work with the referral team to assign high risk patients to a
therapy and/or psychiatric provider and continue to follow up with
and engage the patient until their first visit with a clinic based
program.
In addition, this position will be required to:
- Provide individual counseling sessions remotely using the Doxy
platform.
- Ensure all clients are checked in as part of the billing process.
- Must be willing to work nights, weekends, or when special events
and/or presentations are scheduled.
- Attend all scheduled meetings.
- All other duties as assigned.
QUALIFICATIONS
To perform this job successfully, an individual must be able to perform
each essential duty satisfactorily. The requirements listed below are
representative of the knowledge, skill, and/or ability required.
EDUCATION AND/OR EXPERIENCE
- Masters in Social Work and/or Counseling
- Must be able to provide documentation of a being a Licensed Clinical
Social Worker (LCSW) and/or Licensed Clinical Professional Counselor
(LCPC)
- Excellent written and verbal communication skills.
- Computer literacy and ability to utilize various software programs.
- Ability to arrive on time via own transportation.
- Must be able to accept and incorporate constructive feedback, be
flexible in scheduling, and have concern for youth with a strong
desire to work in the human service arena.
OTHER QUALIFICATIONS
- Current drivers license
- Current auto insurance
- Reliable transportation
- Valid BLS/CPR
- Bilingual skills (English/Spanish) a plus
- Ability to work late evening hours, weekend hours, and scheduled
holidays
