[ General Purpose:]{.underline}

The Smart Living Professional reports directly to the Director of Home Care & Hospice and acts as an advocate for the member.  This person may provide direct service to the members of the program.  The position is PRN but hours may increase over time and become a part time or full time employee -- HR will monitor hours. 

 

[Essential Duties and Responsibilities]{.underline}[:]{.underline}

  • Assess member needs
  • May provide care and service coordination
  • Communicate regularly and clearly with family/responsible party as appropriate
  • Provide home management services which may include but not limited to assisting with:
    • Bill paying
    • Medical forms
    • Correspondence, letter writing, e-mail, sorting papers
    • Travel arrangements
    • Technology
    • Decision making
    • Organizing and planning
    • Implementation/coordination of plans
    • Anticipating issues, problems, concerns and notifying others
    • Personal shopping
    • Meal planning/preparation
    • Transportation
    • Companionship
    • Accompany to events
    • Help with packing and moving residence
    • Caring for member's plants and animals
    • Travel companion on limited trips -- must be arranged in advance
  • Perpetually re-assess the needs and services with members/family/responsible party
  • Maintain accurate daily records of services provided
  • Maintain accurate statistics
  • Communicate clearly and in a timely manner with members of the SLP team
  • Ensure hours and billing are completed timely and accurately
  • Promotes and participates in Quality Improvement initiatives
  • Maintains a record of acceptable attendance and punctuality
  • Observes all company rules and safety procedures
  • Maintains professional appearance at all times
  • Other duties as assigned

[Minimum Qualifications, Education and Certifications]{.underline}[:]{.underline}

  • 2+ years working with older adults and/or individuals with dementia
  • LSW desirable but not required, college experience
  • High energy and positive attitude
  • Valid Ohio Driver's license with less than 3 incidents
  • Well maintained car that provides easy access to elderly passengers
  • Math skills required for bill paying
  • Excellent organizational skills
  • Flexible schedule -- some nights and weekends
  • Computer, verbal, and management skills
  • Ability to productively manage a high level of autonomy

Requires good vision, hearing, speech and communication skills with the ability to ambulate, project voice, use the telephone, use the computer and read and write English. Additional physical demands may include kneeling, climbing ladders, reaching, pushing, manipulating, standing, sitting, and speaking on the telephone for extended periods, crouching, stooping, lifting up to 25 lbs. and repetitive motion.
