Job Description:This position provides clerical and administrative support
to enhance integrated care management for targeted populations and
individuals. Facilitates communication with payers and authorization of
services in accordance with regulatory and contractual requirements, patient
and family-centered interdisciplinary care planning, effective care
coordination, collaboration and communication between healthcare settings,
and effective transitions to the appropriate level of care.
As a Care Management Specialist you need to know how to:
Link patients to specialty care, provide support for the healthcare
providers, office staff and practice administrator.
Coordinate financial needs, surgery schedules and medical needs of patients
from physician to specialist offices as well as schedules medically necessary
procedures for patients.
Assists in managing patient flow, both in office and via telephone
interaction, such that their healthcare provider is enabled to be
informed, organized and efficient with patient care.
Provide patients with cost estimates prior to surgery and advise them on
paying for services.
Educate patients about payment options and financial assistance.
Examine claims and verify insurance eligibility.
Communicate with insurance carriers, update internal billing registrations.
Works collaboratively with secondary prior authorization companies to obtain
appropriate authorizations when necessary.
Appropriately document and report payment information.
Maintain records of medical costs, adjustments, co-payments, and other
billing details.
Reviewing patient records using medical coding procedures.
Minimum Qualifications
High school diploma or GED equivalent
Preferred Qualifications
Bilingual Spanish speaking
Physical Requirements:
Interact with others by effectively communicating, both orally and in writing.
Operate computers and other office equipment requiring the ability to move
fingers and hands.
See and read computer monitors and documents.
Remain sitting or standing for long periods of time to perform work on a
computer, telephone, or other equipment.
May require lifting and transporting objects and office supplies, bending,
kneeling and reaching.

The hourly range for this position is listed below. Actual hourly rate
