WorkSource Oregon Clinic Community Health Worker in EUGENE, Oregon

Position Summary This position works closely with our clinical staff to improve access to treatment for target population. The Clinic Community Health Work (CHW) will assist our clients accessing medical treatment, behavioral health, lab services, medication and medication storage and other clinical care as needed. This position works with the patients to engage in care, communicate their needs, and follows-up on treatment progress. The work will be primarily short-term assistance overcoming immediate barriers to care. The role of the CHW includes assisting the client in accessing medical programs. This includes Medicaid, Medicare, COBRA/Portability, private insurance, and Pharmaceutical Patient Assistance. Ongoing advocacy and support is provided to the client in order to maintain access to medical assistance programs, address barriers to care, and minimize any break in access to medical care. The CHW will also provide phlebotomy services in the clinic. Experience is preferred but not required. Training will be provided. This position will assist clients in accessing private and government social services to meet basic needs such as housing, transportation and food, as well as alcohol and drug programs, mental health services, vocational programs and SSI/SSDI. The HCV Case Manager facilitates this access through the provision of referrals, and additional coordination or advocacy as needed. Essential Functions and Responsibilities Provide phlebotomy services Provide transportation and accompany clients to appointments to facilitate access to treatment and other services and resources Navigate clients to additional services and resources, including health insurance, basic needs services, and housing assistance Provide direct one-on-one and phone care coordination services Make appropriate referrals, including follow up and documentation. Advocate with other case managers and other service providers on behalf of our clients Health education, including explanation of available services, and partner programs Monitoring of service delivery and reassessment of needs Maintain up to date information regarding other benefits and entitlements programs available in Oregon.
