Social Worker MSW - Home Health at Hospice and Home Care of Snohomish County
Schedule: Day Shift, 8:00 am - 4:30 pm, M-F
Job description:
This position is responsible for providing referral services to home care clients, families and caregivers, addressing issues such as adjustment to illness/prognosis, care system issues, community resource issues, and insurance/financial issues. The MSW develops and implements a plan of care in collaboration with the Home Care team providing care to the patient.

Required Qualifications:
Master's Degree Social Work
Upon hire: Washington Advanced Social Worker License Or
Upon hire: Washington Advanced Social Worker Associate License Or
Upon hire: Washington Clinical Independent Social Worker License Or
Upon hire: Washington Clinical Independent Social Worker Associate License
Within 30 days of hire: National Provider Identifier (NPI) number
1 year Health care setting.
