GENERAL DESCRIPTION
This position is part of the Hospice health team. This position is
responsible for the health care needs of Hospice patients providing
skilled nursing care, affording them the opportunity to remain at home
and receive the medical care required.

ESSENTIAL DUTIES
- Completes assessment and develops plan of care appropriate to
patients health care needs. May perform client assessments, when
requested.
- Adheres to the established client care plan and the Physicians Plan
of Treatment. Makes ongoing additions to the clients care plan, as
needed. Follows up with executing and properly documenting any new
doctors orders.
- Performs assigned duties accurately and efficiently to ensure the
highest patient safety, including administration of medication,
treatments, and procedures per policy, monitoring the clients
condition, waived testing (glucometer and PT/INR),
tracheostomy/ventilator care, and follows safety and infection
control measures, etc.
- Directs, supervises, and evaluates the performance of Home Health
Aides, as applicable.
- Maintains positive inter-personal relationships with co-workers and
other members of the health care team and the organization.
Exhibited by timely and appropriate communication with the physician
and other health care team members regarding patients needs,
progress, and any changes in condition.
- Creates and supports a safe and welcoming presence for patients and
their families, evidenced by client satisfaction.
- Performs all duties, services and documentation in full compliance
with CHC/SEK policies and procedures.

- Ability to travel as needed.
- Complies with the agency dress code, projects a professional
attitude toward self, patients, and other staff members, and follows
departmental public relations policies regarding outside speaking
and professional affiliations, clearing all such requests through
the agency.
- Maintain patient confidentiality in accordance to organizations
policy and procedure and HIPPA requirements.
- Social and cultural sensitivity appropriate to ethically and
economically diverse patient and employee base.
