Summary:

The Social Worker II is assigned to specific clinical care units or specific patient populations to provide services to patients and families whose medical, psychological, and/or social situation impacts compliance with the physician care plan, the provision of appropriate health care services for the patient, and/or timely discharge. The Social Worker coordinates the assistance necessary to meet patient-specific needs relative to placement, basic living resources, home health resources, therapeutic counseling, and post hospital support in order to optimize outcomes. Working collaboratively with the Care Manager and other patient care team members, the Social Worker serves as an advocate to ensure that the patient’s physical, psychological, and social needs are identified and met in a timely manner, and that rights are respected and addressed. The Social Worker avails self to all members of the health care team for consultation and intervention.

Responsibilities:

  1. Attends and participates in Communication and Patient Planning (CAPP), daily huddles, and length of stay meetings. Provides and receives information for assigned cases. Actively participates in problem solving.

  2. Identifies and prioritizes urgent or high-risk patients who may require social work, discharge planning or care management services. Provides initial and ongoing assessments for patients and families having complex psychosocial needs and/or require discharge planning to meet their continuity of care needs. Assessments include emotional status, functional capacity, patient safety issues, support systems, and available resources.

  3. Develops treatment goals and plans interventions that assist the patient and family to resolve psychosocial concerns that may arise as a result of illness, engagement in high-risk social behaviors (i.e., substance abuse), limited finances, patient safety, and or physical disabilities. Develops plans, in collaboration with the patient/family, based on the assessment of the patient’s family’s needs, their strengths, and available resources.

  4. Implements social work interventions that address identified problems and facilitates the patient’s movement to the next level of care as soon as medically indicated. Provides patient and family education to support progression of care. Assumes a level of “expert” knowledge of community resources (social agencies, groups etc.) and provides education or referrals to support the patient and family.

  5. Utilizes complex decision making and problem-solving skills that facilitate developmentally appropriate patient plans, interventions and outcomes.

  6. Maintains timely and effective oral, electronic and written communication.

  7. Maintains current knowledge of information related to medical social work, discharge planning, and case management through review of current literature and, attendance and active participation in continuing education, in-service trainings and professional organizations.

  8. Serves as a leader within the interdisciplinary team and demonstrates the ability to manage work relationships in a manner which enhances professional role and departmental effectiveness.

  9. Actively participates in activities which support and enhance departmental and organizational goals to include the performance improvement process, program/policy development and activities related to TJC standards.

Other information:

Education

MSW from a school of social work accredited by the Council of Social Work Education required. Will consider concurrent enrollment in a MSW program and ten (10) years of relevant experience.

Licensure/Certification

LCSW or Case Management certification preferred.

Experience

Two years’ experience including case management or related clinical experience preferred.

Knowledge, Skills and Abilities

• General knowledge of the health care system and patient care resources within the hospital and the community.

• Knowledge of applicable local, state and federal laws.

• Ability to maintain collaborative working relationships with patients, significant others, teammates, medical staff and the general public.

• Outstanding interpersonal skills.

• Organizational, critical thinking and time management skills.

• Knowledge of the age-specific needs of neonatal, pediatric, adolescent, adult and geriatric populations and awareness of cultural differences.

• Ability to read, write and communicate effectively in English.

• Proficient with MS Office and able to learn new software rapidly. Experience with EPIC preferred.

• Flexibility to provide coverage in a department that operates 7 days a week as needs arise.

Valid NC Driver’s License: No

If driving a Wayne UNC Vehicle, must be 21 years old and MVR must be approved by Risk Management.
