Mayfair Care Center, part of The Grand Healthcare System is seeking a Full-time Staff Social Worker to join our team!
Duties Include:
Interviewing participants and their families
Provide psychosocial assessment and counseling to participant and families
Developing specific care plans based on the needs of individual patients
Helping participant apply for appropriate social services
Providing crisis intervention as needed
Providing grief and loss counseling and supportive counseling
Strengthening communications among participant, families, and the program or facility staff
Requirements Include:
A current/valid Bachelors Degree
Long Term Care exp. is a plus
NYS Screen Certification preferred
Strong admission and discharge planning experience
We offer the Following:
Salary $50,000.00 - $55,000.00 per year
