Job Description:The Behavioral Health Care Manager Coordinates the care and
services for patients identified as needing assistance or meeting Case
Management criteria. Provides direct and comprehensive care to patients
identified with behavioral, chemical dependency and/or mental health
needs. Works collaboratively with physicians and other members of the health
care team to improve the health of patients with chronic conditions or complex
needs. The position educates patients and caregivers to help them manage their
health care needs. The incumbent facilitates communication, coordinates
services, addresses barriers, and promotes optimal allocation of resources
while balancing clinical quality and cost management.
Scope
As a Social Work Case Manager you need to know how to:
Coordinate the care and services for patients identified as needing assistance
or meeting care management criteria.
Assess the patient's prior level of functioning, access to and/or use of
community resources and available support systems.
Identify appropriate admission and continued stay issues.
Attend, facilitate, and participate in rounds and case conferences.
Conduct comprehensive behavioral health assessments for patients on mental
health hold in the ED and on medical floors when ordered by a physician.
Participate in in multidisciplinary care planning meetings.
Communicate and collaborate with medical, interdisciplinary team staff,
patient, family, insurance companies, and social agencies to provide
quality discharge plan to appropriate, safe, level of care.
Provide community resources for psychosocial, financial, and educational
needs of patient and family.
Minimum Qualifications
Master's Degree in Social Work from an accredited program, required
Successful completion of LCSW exam and current clinical supervision,
required. LCSW, required within three (3) years of hire
Minimum of two (2) years of clinical experience, required
Current LCSW license, preferred
At least two (2) years of case management experience in an acute care
setting, preferred
Physical Requirements:
Ongoing need for employees to see and read information, labels, assess
patient needs, operate monitors, identify equipment and supplies.
Frequent interactions with patient care providers, patients, and visitors
that require employee to verbally communicate as well as hear and understand
spoken information, alarms, needs, and issues quickly and accurately,
particularly during emergency situations.
Manual dexterity of hands and fingers to manipulate complex and delicate
equipment with precision and accuracy. This includes frequent computer use and
typing for documenting patient care, accessing needed information,
medication preparation, etc.
May be expected to stand in a stationary position for an extended period.
