Summary:\ The Care Coordinator will act as a liaison and coach for selected patients identified to be at-risk. The Care Coordinator will coach the patient, ensure that patients understand and are adhering to instructions, encourage self-engagement with focus on achievement of goals. The Care Coordinator will identify gaps and overlaps in the provision of collaborative health services. The Care Coordinator will demonstrate skills needed to develop capacity for improving health status. All other duties assigned.
Other information:\ Registered Nurse in the state of AR required. Bachelor\'s Degree in Nursing or equivalent is preferred. Certification in Case Management will be recommended in a specified time frame. Previous case management experience or related field preferred. Requires good interpersonal and communication skills.
