DESCRIPTION/RESPONSIBILITIES: Work Schedule : Full time 1.0FTE,
temporary position. Rotating Days/Weekends/Holidays. Day/Evening Shifts
ranging 6:00 AM - 11:00 PM. Hours may vary based on the operational

evidence-based medication therapy management services. * Collaborate
with other providers to assure drug therapy is safe, effective,
efficient and cost-effective. * Manage and improve the medication-use

east side - provides post-acute inpatient rehabilitation care in a
free-standing rehab hospital. The 50-bed hospital offers short-term
inpatient rehabilitation services and education for people recovering
from an injury or an illness and will specialize in helping patients
regain their ability to perform daily tasks, regain cognitive processes
and physical function, restore basic life skills and facilitate
psychosocial adjustment and return to the community. UW Health Pharmacy
Services Building - a non-clinical location in Middleton that houses
nearly 100 Pharmacy professionals. These employees perform various
services and duties, including pharmacy administration, Pharmacy
Technician Training Program, pharmacy primary care services, medication
prior authorization, sterile and non-sterile compounding, specialty/mail

EXPERIENCE AND QUALIFICATIONS: Education : Minimum - PharmD degree or
equivalent experience Work Experience : Preferred - Prior pharmacy
related work experience Licenses and Certifications : Minimum - Licensed
or eligible for licensure in the State of Wisconsin. *****
