JOB REQUIREMENTS: Clinical Pharmacist - Transplant - 31665 Work Schedule
: 100% FTE, Day/Evening, 7:00am - 3:30pm/8:00am - 4:30pm, with rotating
evening, 1:30pm - 10:00pm, and every 4 th weekend. Hours may vary based
on department needs. You will work at University Hospital in Madison,

Pharmacist - Transplant to: * Provide medication therapy management
services as part of the interdisciplinary transplant team to patients on
inpatient and ambulatory services to assure that drug therapy is safe,
effective, efficient, and cost-effective. * Perform both clinical and
distributive tasks necessary to ensure that patients\' medication
therapy is safe, effective, timely, and efficient. * Provide training
and education to Pharmacy Residents and PharmD students through
practical and didactic experiences. * Provide patient care to a diverse
population including kidney, liver, pancreas, heart and lung transplant

EXPERIENCE AND QUALIFICATIONS: Education : Minimum - Bachelor\'s degree
in Pharmacy or a PharmD degree Preferred - Completion of an ASHP
Accredited PGY2 Transplant Residency training program Work Experience :
Minimum - One year of clinical patient care and orders management
experience Preferred - Three years of institutional experience (hospital
or integrated health system) or completion of an ASHP accredited PGY2
Transplant Residency training program Licenses & Certifications :
Minimum - Licensed to practice pharmacy in the State of Wisconsin or
must obtain within 90 days of hire date Preferred - Certification
through the appropriate certifying program (Board of Pharmacy
