JOB REQUIREMENTS: The Intake Specialist supports CLA\'s mission by
performing all tasks related to referrals for all CLA offered services.
The position coordinates referral process by monitoring communications,
providing information about CLA service availability, and supporting
referral action steps with internal and external partners. Ensures
accurate maintenance of referral data and providing reports as needed or
requested to the leadership team. Position assists CLA Team with
marketing outreach initiatives, as needed. Conducting initial screens
for individuals, their surrogates, or long-term care funding connection
interested in CLA. Coordinates with Program Managers to assist with
client enrollment. Replying to inquiries from potential clients to
identify programs and services offered by CLA. Maintain data and provide
reports related to intake and referral activities. Operate office
equipment, such as voice mail messaging systems, and use word
processing, spreadsheet, or other software applications to prepare
reports, invoices, financial statements, letters, case histories, or
medical records. Coordinate communication between patients, family
members, medical staff, administrative staff, or regulatory agencies.
Refer patients to appropriate health care services or resources.
Maintain knowledge of community services and resources available to
patients. Explain policies, procedures, or services to patients using
medical or administrative knowledge. ***** OTHER EXPERIENCE AND
QUALIFICATIONS: Two (2) years experience with long term care supports
and services, program intake, and enrollment, required. Two (2) years
experience in customer service, public communications, community
