We offer a variety of programs including inpatient and outpatient behavioral health, residential treatment for court-involved teenage girls, geriatric services, clinical day treatment schools and chemical dependency services. Through our multi-site network of care, we constantly strive to meet its mission in helping people find their way while educating and empowering individuals to participate in their own care and recovery. _Job Summary_ The Case Manager is responsible for providing a range of services for clients with either Psychiatric and/or Substance Abuse issues. Also provides advocacy, outreach, and service linkage for clients and initiates access to entitlement services. * Works collaboratively with the Therapist and other members of the multidisciplinary treatment team to coordinate aftercare arrangements for the client. * Facilitates groups as required * Ensures personal, client and unit/staff safety. • Experience working with a behavioral health and/or chemical dependency population • Bachelor's degree in Human Services or related field, Social Work preferred • Valid driver’s license may be required based on program • Knowledge of community resources preferred. • Computer proficiency and keyboarding skills required We take great care of careers.
