| needs of those who could benefit | |
| from a more personalized service. | |
| MYPAC staff partner with | |
| families, youth, and community | |
| resources allowing families the | |
| most successful outcome possible | |
| within their home and community | |
| setting. This program provides an | |
| alternative to traditional | |
| psychiatric residential | |
| treatment. | |

| Work hours are | |
| Monday-Friday, 8a-5p; Some | |
| on-call hours and on-call | |
| responsibilities required. | |

| 100,000/300,000/50,000 within | |

| Responsibilities: | |

| services to PBMHR consumers. | |
| - Provides outpatient social | |
| services linkage, emergency | |
| intervention and crisis | |
| intervention services. | |
| - Provides services for the | |
| family through home, school, | |
| or other community visits. | |
| - Documents all services | |
| provided through electronic | |
| medical records by using | |
| collaborative documentation. | |
