Job Profile Summary

​This role focuses on providing medical and clinical support services to patients. In addition, this role focuses on performing the following Community Health duties: Delivers training, facilitates sessions, and educates the community on a variety of clinical topics. An organizational related support or service (administrative or clerical) role or a role that focuses on support of daily business activities (e.g., technical, clinical, non-clinical) operating in a “hands on” environment. The majority of time is spent in the delivery of support services or activities, typically under supervision. A senior level role that requires broad knowledge of operational procedures and tools obtained through extensive work experience and may require vocational or technical education. Works under limited supervision for routine situations, provides assistance and training to lower level employees, and problems typically are not routine and require analysis to understand.

Job Overview

This position provides outreach support including strengthening relationships with community agencies and internal staff and facilitating screenings and events. Facilitates bilateral communications with internal programs, clinical departments, external linkages to other community groups and organizations as well as shares information from the community with the health system. Coordinates and facilitates grant programs.

Job Description

Minimum Qualifications :

  1. High school diploma or equivalent.

  2. Two (2) years of experience in community outreach, affairs, and/or community based public health.

Preferred Qualifications :

  1. Bachelor of Science in Business Administration.

  2. Three (3) years of experience in community outreach, affairs, and/or community based public health.

  3. Bi-lingual or multi-cultural.

  4. Experience with facilitating needs assessments, knowledge of local community groups, experience with community health education and promotion, etc.

Duties and Responsibilities : The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.

  1. Works collaboratively with the Community Benefits and Operations Manager to plan for a strategic approach to addressing a variety of community health needs.

  2. Facilitates agency communications with health system to identify opportunities to provide health promotion and outreach.

  3. Coordinates system-wide outreach to vulnerable populations in the community including annual holiday drives.

  4. Develops relationships with diverse community-based organizations, by participating in coalitions, and representing the health system at community screenings and events.

  5. Provides group facilitation and program oversight including documentation and reporting and record keeping requirements.

  6. Manages a multi-session grant funded program including outreach, recruitment, facilitation and documentation.

  7. Provides resource and referral information and supports the overall procurement of supplies and materials needed to operate the Mother’s Helping Mothers family support program.

  8. Assists with community benefits reporting and documentation and provides support to focus group and community forum work to meet regulatory requirements.

  9. Prepares and edits documents and reports for grants and supports the Community Services department in preparing other important documents such as the annual community benefit report.

  10. Supports and mentors new community health workers across the health system.

  11. Orders and catalogs supplies for three Community Services departments.

  12. Collaborates or conducts outreach with other Community Services departments, which includes WIC, NSCFRN and Community Health Education.

  13. Supports activities aimed at improving health in the communities by promoting early detection, health education and access to resources.

  14. May travel to community or agency meetings.

Physical Requirements :

  1. Frequent sitting, occasional standing, walking, and lifting of 10-20 lbs.

  2. Requires fairly rapid manual dexterity using fine hand manipulation to operate computer keyboard.

  3. Requires ability to clearly read, write and comprehend written materials/computer screen and reports.

  4. Auditory ability at normal speaking level, to send and receive phone messages, etc.

Skills & Abilities:

  1. Knowledge of Microsoft Word, PowerPoint, Excel, Outlook (Meditech a plus).

  2. Excellent written and verbal communication skills.

  3. Proven administrative skills and experience.

  4. Superior interpersonal and organizational skills.

  5. Ability to work independently, problem solve and negotiate, have critical thinking skills and work with diverse populations.

  6. Ability to work non-traditional hours including nights and weekends occasionally.
