Description

Job Purpose: This is a professional position that is responsible for the care and treatment of mentally ill adults. The Case Manager will develop and implement the treatment plan, which will include the necessary follow-up, case management and intervention as required.

Reports To : CCS Clinical Coordinator

Essential Job Functions:

  • Provide psychosocial rehabilitative services for assigned clients; documents clinical files for all client contacts; consults with clinical staff on concerns and client needs.

  • Works as part of team to provide a treatment program for client, acts as advocate with appropriate agencies for individual clients.

  • Monitors client activities and facility environment; helps to create a safe and therapeutic environment for clients; follows established safe practices; protects clients from personal injury; follows safety protocols and calls for assistance as required.

  • Intervenes as necessary to prevent harmful behavior and de-escalate clients, monitors, reviews client’s physical and emotional stability.

  • Develop and enhance cooperative professional relationships with local community agencies significant to the continuum of care for clients.

  • Updates and maintains a variety of files, records, charts and other documents; gathers, compiles and organizes data for management purposes; maintains appropriate records and prepare reports both verbal and written.

  • Assists consumers in learning independent living skills, which are defined in the resident’s Recovery Plan of Care. (i.e. laundry, housekeeping). As well as encouraging learning about symptoms and medications through support and education.

  • Monitors consumer medication regime and compliance as directed by their physician when prescribed in the Recovery Plan of Care.

  • Reports to appropriate staff any changes in consumer behavior and functioning.

  • Familiarizes self with client Recovery Plan of Care.

  • Supervise and document client groups as appropriate.

  • Attends and participates in staff meetings, in-service training seminars and conferences as required.

  • Client transportation is a requirement for this position.

  • Access to a personal vehicle.

Required Education, Experience, Certifications, Licensure and Credentials: (Where appropriate, education and/or experience may be substituted)

Minimum Required Education: Bachelor's Degree is required in the field of Social Work, Counseling, or related Human Services Field.

Minimum Required Experience: Minimum of 1 years’ experience working with adults with severe and persistent mental illness.

Required License – Certification – Registration :

Motor vehicle record acceptable to program and/ or contractual requirements.

A valid Wisconsin driver's license, and automobile insurance with minimum limits of

$50,000/$100,000 bodily injury and $10,000 property damage”.

Certification as a functional screener is not required but desired.

Knowledge – Skills – Abilities:

  • Must be familiar with Microsoft Office, such as Word, Excel, Outlook, and Teams.

  • Professional workplace appearance and conduct; friendliness and courtesy to clients, coworkers, and vendors.

  • Effectively communicates and develops good working relationships with co-workers.

  • Reliability in reporting to work regularly and on time.

  • Must possess good communication, listening, writing and organizational skills.

  • Ability to effectively train coworkers.

  • Experience with adults undergoing a psychiatric crisis.

  • Knowledge of community resources available to consumers in the Milwaukee County area.

  • Timely, informed decisions that consider the facts, goals, constraints, and risks.

  • Partners with clients to identify and resolve complex or sensitive issues.

  • Adheres to all workplace and trade safety laws, regulations, standards, and practices.

  • Builds and maintains customer satisfaction with the products and services offered by the organization.

Physical Requirements, Visual Acuity, and Work Conditions:

Physical Requirements : On occasion must be able to push/pull/lift/move objects weighing at least 50 pounds. Must have the ability to climb at least two flights of stairs.

Visual Acuity : The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal.

Working Conditions : Work is performed largely in an office environment or other various settings which may include various outdoor weather conditions, loud noises, and fluctuating temperatures and the occasional use of stairs. This position frequently works with office equipment including, but not limited to, personal computers, photocopiers, fax machines, and multiline phones.
