University of Minnesota Physicians RN Care Coordinator (Orthopedic Clinic - CSC) - .8 FTE in Minneapolis, Minnesota
As the multi-specialty group practice for the faculty of the University of Minnesota Medical School, University of Minnesota Physicians has more than 950 physicians and 1,600 health professionals, providing clinical care in over 100 specialty and sub-specialty areas. As a Care Coordinator – RN you will provide a deliberate patient and family centered partnership with the healthcare team by developing and supporting a coordinated plan of care between visits (Inpatient and Outpatient) to promote a smooth transition for patients and families across the healthcare continuum.
The Care Coordinator - RN works to maximize the patient’s health outcomes and experience by identifying patient and family needs, completes pre-visit planning, supports clinic visits, and provides closure to the clinic visits. The Care Coordinator – RN utilizes coaching to help ensure patient and family participation in treatment plans and provides exceptional customer service to internal and external customers (providers, clinic staff, patients, and families).
Care Coordinator - RN Overview
Key responsibilities include but are not limited to:
Coordinates care across the health care continuum within designated sub-specialty population(s)
Serves as single point of contact by facilitating and overseeing transitions in care within and between care settings, providers, and services
Completes pre-visit planning and post visit wrap-up and determines necessary follow-up appointments and/or treatments to optimize patient visit
Facilitates proactive disease management and preventative health care using evidenced based practice
Performs other duties as determined by manager and clinic needs
Current Minnesota RN license and completion BSN at time of hire
3+ years of clinical nursing experience
Demonstrated experience in patient advocacy and education, coaching, and proactive disease management
EMR and keyboarding/computer skills
