Employment Support Professional/Job Coach II Position Description As a key program staff the individual in this position will assume the following responsibilities: Program design, implementation and evaluation. Will serve as the primary supervisor at community-based job sites as assigned. Will implement employment support services and other assigned programs directly with program associates, ensuring person-centered, individualized, positive, high-quality, supportive environments. Will promote and assure treatment of associates in workplace equal to that of other employees. Assist program associates in augmenting skills and abilities which promote their success Perform comprehensive job and task analysis: Will provide non-vocational classes, instruction, community skills, and support as assigned. Will evaluate program effectiveness, making adjustments to programs when necessary. Will assist program associates in fulfilling objectives and outcomes identified in their Community Support Plans (CSP) and Coordinated Services Supports Plans (CSSP). Assure health, safety and rights of program associates during participation in eQuality. Know program associates health concerns. Maintain requirements of the Vulnerable Adults Law. Assist in coordinating and providing associate transportation as required. Provide high quality services to eQuality constituents. Ensures that eQuality associates vocational planning and delivery is person-centered and that opportunity to explore, work toward, or secure competitive, community-integrated employment is available to all. Identify and support associates preferences in relation to what is important to and for each individual. Utilize tools and information to assist associates to identify their desired outcomes. Deliver all services in a manner that is respectful and culturally appropriate to each individual served Encourage informed choice, self-determination and active participation throughout all services Emphasize associates strengths, interests, and talents. Fulfill eQuality s commitments to employers by ensuring that assigned tasks are completed and the expectations of employers are met. Complete all documentation required accurately and in a timely manner, including but not limited to: (1) Daily progress and outcome reports (2) Vocational and skills assessments (3) Productivity assessments (4) Timesheet and attendance information for assigned associates (5) Other documentation as requested or assigned (6) Acts as Designated Coordinator for assigned associates, complying with all licensing and organizational requirements inherent in this designation. Achieves associate growth and development through individualized program plans. Meets as needed with associate teams. Solicits and incorporates feedback from individuals and teams into plans as much as possible. Communicates effectively with associate teams. Reports accurately and as required to associates and their teams. Organization/Community Initiatives and Leadership: Develop positive relationships with local employers, County Case Managers, program associates and their families. Follow and enforce all organizational policies and procedures Adopt necessary measures to ensure that all organizational initiatives comply with the mission Complete all other assigned tasks as may be necessary for the organization to fulfill its commitments and mission
