UNC Health Care PHARMACIST - POPULATION VALUE CARE in Morrisville, North Carolina


Position: Pharmacist / Population Value Care

Location: Think Place / Morrisville

Hours: Full Time, 40 hrs/wk; Days

This position may involve support of various hospitals and health care systems within the UNC Health Care System, but will be employed by Rex Hospital, Inc. (this includes, but is not limited to, for purposes of payroll, health benefits, retirement options, and applicable policies).

The Carolina Assessment of Medications Program (CAMP) Pharmacist will report directly to the Clinical Coordinator for CAMP Services. Our team is within Managed Pharmacy Solutions under the UNC Healthcare Department of Pharmacy.


I. Clinical Program Operations (80%)

a. CAMP Clinic—participate in clinical activities for a variety of beneficiaries enrolled into several distinct health plans or programs. Work completed telephonically, virtually, or in face-to-face clinic settings.

i. Clinic Recruitment: select target patients for recruitment based on eligibility criteria and contact patients for enrollment ii. Comprehensive Medication Management: perform visits using the individualized Medication Assessment & Planning iii. Targeted Chronic Disease Management: perform visits focused on core disease states iv. Transitions of Care: assist patients with medication reconciliation and education between care transitions v. Program Development Support: assist in developing work stream documents, tools, and resources for new programs within our group vi. Marketing: assist in promoting the preferred UNC Health Care System pharmacy network II. Teaching (10%)

a. Provide experiential instruction to UNC ESOP PharmD candidates, and UNCH PGY1 and PGY2 pharmacy residents

b. Contribute to the education and mentoring of the UNCH’s Ambulatory Care and Geriatrics Specialty Residents

c. Actively engage in scholarly activities with other Ambulatory Care Pharmacist Practitioners including attendance of Population Health POD

d. Provide didactic instruction to the UNC ESOP students

III. Clinical Outcomes Research (5%)

a. Develop, collect, and report therapeutic intervention and outcomes data to track quality indicators and cost savings for patients receiving prescriptions through covered health plans

b. Coordinate all research activities with team members (from UNC ESOP) including research design, protocol maintenance, and manuscript development

IV. Committee Engagement (5%)

a. Participate in Pharmacy & Therapeutics Formulary Subcommittee and Ambulatory Pharmacy Care Practice Council


• Ability to embrace and adapt to change quickly

• Proficiency with Microsoft Office applications (particularly Outlook, Excel, Word, Visio), technology utilization, and Epic/electronic medical records programs

• Exhibits evidence of being both a self-motivated, independent worker and a valuable team contributor

• Detail oriented, with a high level of organizational skills

• Bilingual Spanish/English


Bachelor's degree in Pharmacy.

Eligible for licensure as a Registered Pharmacist in NC. Licensure from NC Board of Pharmacy required within 6 months of orientation date or sooner if required for operations.
