Imperial Healthcare Rehabilitation and Nursing MDS Coordinator in Neptune City, New Jersey

Requirements: Registered Professional Nurse with at least one year's experience in the Long Term Care setting and MDS.Job Responsibilities:1. Coordinates the development and completion of the MDS in accordance with current rules, regulations, and guidelines that govern the resident assessment, including implementation of RAPs and triggers.2. Maintain written policies and procedures that govern the development, use, and implementation of the resident assessment process.3. Participates in the development, implementation, and maintenance of an ongoing Quality Assurance program for the resident assessment/care plan.4. Prepares the CMC Quality Indicator Report and monitors the triggered indicators and correlates to accurate MDS coding.5. Participates in facility surveys made by authorized government agencies.6. Ensures that the Physician's Medicare Certification and re-certifications are signed per regulation.7. Audits medical records documentation to validate MDS coding.8. Assists in the preparation of all requests from appropriate state and/or federal regulatory agencies regarding payment of services (denials, appeals, UMR, etc.).9. Assesses and evaluates the outcome of the MDS process to determine additional training, education, and monitoring needs.10. Works with the Interdisciplinary Care Plan Team to assure the development of accurate comprehensive resident assessment and care plans.11. Provides oversight for the effective and timely utilization of the care plan team and conferences.12. Performs concurrent MDS reviews to insure appropriate RUGs categories are assigned through the capture of appropriate clinical information.13. Ensures timely completion of all MDS assessments including significant changes.14. Maintains tracking system for admission/discharge to facilitate submission of tracking forms.15. Oversees Restorative Nursing Program
