The Intensive Mobile Treatment (IMT) Team provides treatment and support services for individuals with frequent interaction with the mental health, substance abuse, criminal justice and homeless services systems and have been poorly served by traditional treatment models. IMT teams stay connected to individuals regardless of their location in shelter, on the street, in housing, hospital or jail.
Purpose of Position:The Registered Nurse is responsible for delivering direct and comprehensive nursing care for the IMT team clients which includes overseeing and monitoring their medication management. The RN is responsible for improving the overall health of clients, including promotion of physical and mental health integration. The RN will practice according to the principles of recovery oriented and trauma informed service delivery.
Schedule: Monday through Friday 9:00 a to 5:00 p
Salary Range: $99,134 - $111,525
Roles, Responsibilities, and Essential Duties
1) Medication management
Communicate with team psychiatrist/PNP about client prescriptions
Monitor, dispense and document client medications
Act as liaison between psychiatrist/PNP and pharmacy
Maintain storage and documentation of onsite client prescriptions and medications.
2) Nursing services
Assess psychiatric and medical status of clients
Perform medical assessments and health screenings
Provide referrals to primary and specialty care
Educate clients on health and wellness
Perform blood draws
Monitor and administer psychotropic injection consistent with client preferences and practice standards
Visit clients in residential facilities, on the street and in other locations
Maintain up-to-date client case records according to program guidelines
3) Participate as an IMT team member
Attend staff meetings, case conferences, and training programs to upgrade skills and knowledge
Attend supervision meetings to review client progress and case management strategies
Work collaboratively with IMT team staff in a team case management model
4) Other duties as required
Qualifications/Educational Requirements
Active NY State License as a Registered Professional Nurse
2- 3 years of nursing experience preferred
Driver’s License preferred
Knowledge, Skills and Abilities
Experience working with people with histories of substance abuse, mental illness, homelessness and trauma required
IMT Team experience preferred
Fluency in Spanish a plus
Computer Skills: To Perform this job successfully, an individual should be
Able to work in the Microsoft Office Suite, should especially be proficient in Word, Outlook, and Excel
Able to use or learn to use AWARDS database
Able to use or learn other electronic databases, programs, and funder-related software as needed such NIMRS and Maven
Physical Requirements/Working Conditions
Walking, climbing stairs
Field work required; Must be able to travel throughout the 5 boroughs
