Summary:

At the direction of the Care Manager helps to facilitate the discharge of patients from the hospital to home or extended care facilities by coordinating various processes and tasks related to discharge planning and resource utilization this may include: electronic nursing home listing process, ordering durable medical equipment, checking insurance coverage, obtaining insurance pre-authorizations for needed services, completing state assistance and other applications, transmitting referrals to homecare agencies, arranging transportation, coordinating the final discharge arrangements and coordinating with patients, families and the interdisciplinary team.

Responsibilities:

  1. Nursing Home Placement Process Criteria A: Coordinates referrals and transfers to extended care facilities in accordance with Medicare, Medicaid, HMO Preferred Providers and Case Management Department guidelines. Proactively initiates tasks to ensure the efficient movement of the patient through the hospital system. Nursing Home Placement Process Criteria B: Serves as the liaison between the hospital and nursing facility representatives; fosters professional, positive relationships with liaisons. Responds to calls from community resources in a timely manner and answers questions and/or initiates calls to get questions answered promptly. Nursing Home Placement Process Criteria C: Maintains updated listings of nursing homes and serves as expert resource for the department on specialty services offered by nursing facilities. Nursing Home Placement Process Criteria D: Conducts thorough, timely investigation of nursing home options available for complex cases and obtains pre authorizations for nursing home placement as needed.

  2. Discharge Planning Responsibilities Criteria A: Communicates discharge status of patients with Case Managers, physicians, patients and patients' families on an ongoing basis. Discharge Planning Responsibilities Criteria B: Prepares necessary paperwork for discharge, books transportation and completes applications (i.e. Medicaid) thoroughly and in a timely manner. Orders equipment, oxygen or other items related to patient's discharge planning needs. Discharge Planning Responsibilities Criteria C: Obtains pre authorizations for home care, equipment, or other needed services. Discharge Planning Responsibilities Criteria D: Makes homecare referrals in coordination with the case manager.

  3. Documentation / Computer Entry Criteria A: Documents legibly and clearly in MIDAS. Documentation / Computer Entry Criteria B: Other documentation / computer entry responsibilities are carried out accurately and in a timely manner. Documentation / Computer Entry Criteria C: Provides accurate sign-out to weekend case managers. Leaves detailed sign out to other CMA's for planned days off.

Education: HS GRAD/EQUIVALENT

Other Information:

Required: High School Diploma and minimum of five years job-related experience. Some college preferred

Minimum Experience: five years

Desired: Some expertise in the medical field, knowledge of medical terminology, experience working with the elderly and knowledge of community resources preferred. Requires excellent oral and written skills, autonomous functioning, high degree of organization, ability to work on several tasks simultaneously and to work well under stress. Must be a team player and possess critical thinking skills as well as the ability to help problem solve in a variety of unique situations. Sensitivity to the elderly and desire to work with people required. Bilingual ability preferred.
