As our Home Health Aide at CHI St. Joseph's Health, now part of CommonSpirit Health formed between Catholic Health Initiatives (CHI) and Dignity Health, you are the bridge between our hospice department and our community. You’ll deliver care to our patients in their homes-providing them comfort care in their own environment.

Responsibilities
The Hospice Home Health Aide (HHA) assists in providing basic nursing care to patients under the direction, instruction, and supervision of a registered nurse. Job duties may include: assisting patients with personal hygiene, ambulation and transfers, recording vital signs, promoting patients well-being, reporting changes in condition to the registered nurse, performing light housekeeping duties, preparing meals, and assisting in providing a safe and healthful environment for patient care.
Qualifications
High School Diploma or equivalent required.
Certified Nurse Assistant (CNA) in the state of Minnesota (MN) required.
Basic Life Support (BLS) issued by the American Heart Association (AHA) required.
Must have a valid Driver’s License.
Must have current vehicle insurance.
Preferred:
- Hospice experience preferred.
Pay Range
$18.77 - $24.25 /hour
