Minimum Certifications, Degrees, Registry and/or Licenses Required :

Board Certified Adult Geriatric Primary Care Nurse Practitioner (AGPCNP-BC).

Current licensure or eligible in the Commonwealth of PA. Eligible for credentialing by the medical board.

Skill & Job Requirements : The Hospital of the University of Pennsylvania (HUP) has an exciting opportunity for an Adult Gerontology Primary Care Nurse Practitioner to join a new inpatient Geriatric Primary Care Co-Management team.

The AGPCNP must be able to function autonomously and in collaboration with health care professionals to evaluate, treat and manage patients’ age-related health problems. Must possess and develop decision-making skills which allows for accurate assessment, appropriate intervention, and care planning with a holistic approach. AGPCNP must be proficient in the operation of clinical equipment/machines and computers found in the patient care setting. The AGPCNP, while providing quality care across the continuum in collaboration with physicians, nursing staff and other members of the care team, performs comprehensive geriatric assessments, and provides care and patient/family teaching following the nursing process. Also participates in research as appropriate, ensures regulatory compliance, and documents care according to standards.

The role as a AGPCNP is unique, challenging, and rapidly expanding. Responsibilities will include daily rounds as part of the inpatient Geriatric Co-Management Service. The number and type of consults will vary during the day in indication, acuity, and location. In addition to collaborating with other providers, nurses, therapists, and social workers, etc., the AGPCNP will be responsible for delegation of geriatric focused interventions to student geriatric associates. This is a new position and will likely evolve with time as the needs and demands of the service change. The ideal candidate for this position must be flexible and comfortable with change as the program develops.

Excellent critical thinking, organizational, and communication skills, as well as a strong medical/pharmacology/nursing knowledge are essential. Members of the team participate in a full range of quality initiatives, are encouraged to become active members of professional societies like the American Geriatric Society (AGS) and the Gerontological Advanced Practice Nurses’ Association (GAPNA).

All providers will function under the guidelines as designed by the University of Pennsylvania Medical Center medical staff guidelines and are credentialed through that body. The AGPCNP will be held accountable through a formal job description, through the delineation of privileges, and through their agreement with their collaborating/supervising physician who has initiated a Collaborative Practice Agreement. All clinical work hours and associated requirements will be defined and managed by the department, and within the human resource guidelines
