As a Telephonic RN Case Manager/Disease Manager, you'll wear many hats, and work in a variety of environments. Sometimes, you'll interact with members leaving the hospital - possibly with new medications or diagnoses. You may also act as an intermediary between providers and members - serving in numerous roles, such as educator, evaluator, service coordinator, community resource researcher and more. The result? Fewer hospitalizations, ER visits and costly service gaps; and a less stressed, more effective health care system for us all.

* Flexibility to work 8- or 10-hour shifts *

If you are located in Nevada, Arizona or Utah, you will have the flexibility to work remotely* as you take on some tough challenges.

Primary Responsibilities:

  • Provide a complete continuum of quality care through close communication with members via phone or video platform interaction

  • Support members with diabetes, kidney disease, prediabetes and / or asthma condition education, medication reviews and connections to resources

  • Assist members recently discharged from the hospital or Emergency Room learn best disease management self-care practices

Required Qualifications:

  • Current, unrestricted RN license in state of residence

  • Willing and able to obtain a NV state nursing license

  • 3+ years clinical experience in a hospital, acute care, home health/hospice, direct care or case management

  • Proven bilingual Spanish

  • Proven problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action

  • Willing and able to work some evenings and every Saturday

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN)

  • Certified Asthma Educator and/or Certified Diabetic Educator

  • Chronic kidney disease experience

  • Medicaid and/or Managed Care experience

  • Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs

Nevada Residents Only: The salary range for this role is $58,300 to $114,300 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
