Demonstrates understanding of the hospice philosophy/principles and an
ability to clearly articulate these with others. Supports ongoing
management of the plan of care from referral through discharge for
hospice patients and families per regulations and agency standards.
Completes initial nursing assessment of patient/family to determine
hospice needs. Provides complete physical assessment and history of
current and previous illness(es). Initiates and implements plan of care
in collaboration with patient/family with goals and interventions that
promote optimal patient outcomes while maintaining patient dignity,
autonomy and comfort. Coordinates ongoing provision of medications,
durable medical equipment, nursing supplies, and contracted services
with effective cost-management strategies. Anticipates/makes provisions
for after-hours needs. Administers medications/treatments as prescribed
by the physician. Observes/documents response to interventions, promptly
communicates untoward reactions, and modifies plan of care as indicated.
Develops hospice aide plan of care in timely manner and supervises with
revisions in plan of care as appropriate. In collaboration with
interdisciplinary team, provides teaching and guidance to patient/family
regarding interventions for pain and symptom management. Provides
emotional and spiritual support per plan of care. Coordinates plan of
care for patients residing in facilities. Acts as liaison to family
members and facility staff regarding communication of plan of care.
Coordinates patient care with team, family and physician for assigned
caseload. Completes, maintains and submits accurate and relevant
clinical notes regarding patient\'s condition and care. Documents
measurable outcomes related to symptom management. Point of Service
documentation is an expectation of all clinical personnel for quality
care, accuracy, and communication standards. The clinician is expected
to appropriately document at Point of Service, have timely
synchronization of patient data & effectively communicate with the IDT.
Communicates with physician regarding patient needs and changes. Obtains
physicians\' orders as required. Participatory member of IDT meeting.
Attends IDT meeting a minimum of once a month. Provides and maintains a
safe environment for the patient. Keeps administrator informed of
potentially problematic patient/family issues and significant changes in
patient status. Demonstrates excellent communication skills in person,
writing, phone and computer. Provides exceptional customer service.
Manages multiple priorities and demonstrates effective problem solving
and decision-making. Assumes responsibility for personal growth and
development. Maintains and improves professional knowledge and skills
through continuing education and in-services. Provides complete and
timely nursing reports/handoffs to hospice staff, facility staff, and
providers. Participates in on-call (evening and nights), holiday, and
weekend rotation to ensure the agency\'s ability to provide 24-hour
coverage. Salary Range- \$35.00- \$47.50 per hour Shifts: 1. Monday -
Friday 2. Weekend- Friday, Saturday and Sunday 3. Fulltime M-F, 4-12 on
call nurse Qualifications Education Required Associates or better.
