Atlantic Health System Stroke Program Manager in Pompton Plains, New Jersey

Stroke Program Manager

Clinical Responsibilities:

  • Acts as an expert resource clinical practitioner and role model in caring for and assisting with planning, implementing and evaluating care of stroke patients

  • Will assist with stroke code activations to ensure protocols are followed at supervisory level. Will be responsible to assess any process delays and work on PI projects as associated with the supervisory role

  • Advocates for patients and family members, by designing policies and services that promote patient rights and optimal environments of care, building on customer satisfaction

  • Assists in coordination of the tele-stroke program and transfers among facilities

Collaboration:

  • Collaborates with physician leaders, specialists, nursing, service line team members, and other disciplines on stroke program policies, procedures, and protocols to improve outcomes for stroke patients and enhance the healthcare environment

  • Provides coordination and administrative functions for the multidisciplinary stroke steering committee and other site-specific or system-wide initiatives/committees as needed

  • Utilizes the shared governance forum to provide planning for staff stroke education based on new practices & quality improvement initiatives

Data Management:

  • Collects and records data on quality indicators for all stroke patients for the purposes of proactively assessing for variances and communicating with unit, site, and system level leadership

  • Reviews and analyzes data for trends, shares data with stakeholders, and utilizes data for performance improvement initiatives

  • Abstracts and manages data for Get with the Guidelines and the Joint Commission

Education and Outreach:

  • Participates in community and educational outreach activities.

  • Coordinates efforts with Community Health, AHS Stroke Program Managers, Rehabilitation, New Vitality, EMS, NJ Stroke Coordinators Consortium and other stakeholders in educating the community at large.

  • Identifies and facilitates opportunities for community engagement for stroke patients and caregivers i.e. stroke support groups, community events, stroke camp, and health fairs.

  • Assists with the development and implementation of verbal, written, and visual instructions to patients, families, providers, and nurses in collaboration with nursing educators and service line leadership.

Operations/Administrative Responsibilities:

  • Continuously monitors operational efficiency of the stroke program.

  • Responsible for participating in stroke program marketing at their respected institutions.

  • Will review capital resource requirements needed for their program and this information will be communicated with the neuroscience manager.

Quality of Care/Regulatory:

  • Incorporates current stroke AHA guidelines and evidence-based practice into their respected stroke standards of practice.

  • Ensures the implementation of standards of care according to CPG’s.

  • Assesses the need for, plans, and implements quality improvement programs driving stroke quality measures.

  • Utilizes quality improvement data to identify and implement evidence-based interventions that reflect current scientific knowledge; practice and promote continuity of care.

  • Participates in Joint Commission and State Stroke Center certification efforts focusing on compliance of standards.

  • Participates in patient follow up and monitors patient satisfaction scores.

Research:

  • Participates in research provided by the comprehensive stroke center.

  • Interprets and applies new evidence into stroke care practice.

  • Active in the development of QI/PI projects and communicates findings through presentations/publications/ and hospital leadership.

  • Shares and applies current research and literature in specialty area in direct care, educational, consultant and resource roles.

Growth and Development:

  • Demonstrates the commitment to continual learning and professional growth.

Required:

  1. Bachelor's degree required

  2. 3- 5 years clinical experience required. Preferable clinical experience in Neuroscience/Stroke.
