Provide pharmaceutical services to SPH inpatients and outpatients and to
satellite clinics. POSITION TECHNICAL RESPONSIBILITIES Order Entry 1.
Enter medication orders into (or perform final quality control check if
performed by technical personnel) Meditech computer system and initial
each order sheets to indicate pharmacist has reviewed the order(s). 2.
Review all new orders for appropriateness (date, route, frequency,
indication), duplication of therapy, safety (e.g. drug-drug
interactions, drug allergy) 3. Resolve drug therapy problems identified
by intervening with the appropriate physician or health-team member;
change orders when appropriate. 4. Clarify all incomplete, inaccurate,
or unclear orders with appropriate physician or health-team member. 5.
Perform all necessary dosage calculations. 6. Calculate and pursue
dosage adjustments for patients with impaired renal or hepatic function.
7. Perform and document therapeutic interchanges or formulary
substitution on appropriate agents. 8. Document clinical interventions
in computer system. Patient Care 1. Review all patients\' computerized
medication profiles-intervene if a drug therapy problem is identified.
2. Provide drug information for requests immediately affecting patient
care. 3. Perform drug monitoring consults (i.e. pharmacokinetic
consultations). 4. Manage TPN orders-review and document labs and
perform necessary interventions. 5. Perform therapeutic drug monitoring
on appropriate drugs (review and assess appropriate labs and drug
levels; e.g. warfarin, anticonvulsants, digoxin). 6. Review all
patients\' labs and microbiology cultures and intervene with appropriate
physician if necessary. 7. Patient related service documentation (e.g.
performing and documenting 1st dose teaching, diabetes teaching,
warfarin reaching, discharge counseling). Other 1. Guide and perform
final quality control check of duties that may be performed by technical
personnel: drug selection, preparation, packaging, labeling, etc.
including unit dose cart check. 2. Prepare or supervise/check the
preparation of parental sterile products; including injections, IV
admixtures, TPN. 3. Instruct on the proper methods of administering
drugs and precautions/side effects involved in their use. 4. Insure
timely delivery of medications to all patient populations served. 5.
Assist with the training of new department employees. 6. Provide drug
information and pharmacotherapy recommendations to other healthcare
professionals. 7. Maintain proper drug storage, record keeping, and
utilization data as required by state and federal regulations and good
practice standards. 8. Maintain contemporary working knowledge of
pertinent state & federal laws and regulation and hospital and pharmacy
department policies and procedures that govern contemporary hospital
pharmacy practice. 9. Perform inspection of drug stocks at designated
storage areas to verify proper storage conditions, expiration dates,
quantities, security, etc. 10. Read selectively from professional
literature, local policies/procedures and professional practice
standards to stay abreast of new drug entities, precautions involving
drugs, drug shortages and recalls, and legal requirements of pharmacy
practice. 11. Perform as Department of Pharmacy member of the hospital
\"Code Blue\" cardiopulmonary resuscitation team. 12. Document and
report adverse drug reactions. 13. Presence and participation in
designated hospital committees. 14. Provide direct supervision of
pharmacy technicians (1-3) and one intern/student at any time. 15.
Provide community service presentations, drug information, and
prevention programs to consumer groups and patient groups. POSITION
REQUIREMENTS Education: Re uired: PharmD degree or Bachelor\'s degree in
Pharmacy with equivalent experience. Preferred: None Experience:
Required: 1-2 years of hospital experience as a Pharmacist. Preferred:
None Licenses and Registrations: Required: State of Wisconsin license as
a Pharmacist Preferred: None Certification(s): Required: Basic Life
Support (BLS) certification within 3 months of hire and Advanced Cardia
