JOB SUMMARY: Responsible for providing Veterans, spouses/beneficiaries and/or dependents case coordination and case management services to clients and families to include case management, psycho-social and or resource needs assessments and participates in outreach activities and attends collaboration meetings with internal/external stakeholders to reach the assigned special population and serve the communitys needs. Participates in planning of programs, policies or objectives for own work group and department. Works directly with veterans, beneficiaries/spouses/dependents. This is a grant funded position.

DUTIES and RESPONSIBILITIES: Conducts client and/or family psycho-social and/or resource needs assessment and obtains maximum benefit from all available sources. Maintains appropriate documentation, while also writing informational reports, data/information in accordance with professional standards and policies. Develops and implements client/family service plans and monitors progress within grant parameters if applicable. Conducts periodic and final assessments to determine progress, clients barriers, follow-up needs and client outcomes. Provides brief counseling and crisis intervention and motivational interviewing to individuals and/or their families to increase their ability to overcome and prevent crisis and promote progress toward self-sufficiency, health and well-being. Provides short- or long-term case management and service coordination to ensure maximum self-sufficiency and non-institutional living. Utilizes community resources to assist client in learning to live as a self-sufficient member of the community, including but not limited to medical resources, local, state, and federal agencies, community-based agencies and other resources. Participates in community collaboration for resource development to strengthen the clients social service network. Participates in case reviews involving client/family and other service providers. Provides client/family education, training, and advocacy; Serves as an advocate for client. Delivers interventions as needed to client, children, and families with a client centered approach. Participates in evaluative and quality assurance activities. Maintains current knowledge about resource availability, service costs, and budgetary parameters and remains fiscally responsible in carrying out all case management functions and activities. Performs other duties as assigned. Participates in activities and duties related to emergency management during a local state of disaster as directed by appropriate county managers.

REQUIREMENTS: Bachelors Degree in Social Work, Counseling, Cultural Studies, Education, Human Services, Emergency Management, Psychology, Public Administration, Public Health, Public Policy, Public Service, Social Sciences, Sociology, Therapy, or other relevant field of study preferred. Five years of experience can be supplemented in lieu of education. 2 years of experience working with vulnerable populations in the field of social services. Preferred experience working directly with Veterans. Strong written and verbal communication skills. Strong interpersonal skills. Ability to work well on a team to accomplish goals. Ability to work on multiple projects with clear deadlines. Ability and willingness to work with a diverse range of groups, including all communities, ethnicities, religions, and cultural and social groups/backgrounds. Ab lity and willingness to travel into the field and participate in outreach events as needed. Ability to work independently and take initiative. Strong organizational leadership, collaboration, and innovation skill sets. Knowledge/experience working with case management or wrap around services in a social service setting with special populations that include, victims of crime, seniors, children/adolescents, mental health disorders, hospital, homeless, unstably housed, nursing home, schools, advocacy, or agency setting preferred. Strong computer and software skill sets, including Microsoft Office suite. Knowledge/experience working with Caseworthy, HMIS, or other similar case management software, and grant funded programming preferred. Knowledge of current research and social services models/modalities and interventions on both the micro and macro level. Knowledge/experience with psycho-social assessments preferred. Military background preferred; Bilingual preferred.

SALARY RANGE: $24.72 - $30.91 hourly based on qualifications.
