Community Care Network is looking for a Community Rehabilitation and
Treatment Case Manager to join our team!

About the Role:
The Community Rehabilitation and Treatment Case Manager is responsible
for managing an assigned caseload of clients with major, severe,
recurrent and long-term mental health disorders through coordination of
services, provision of supportive counseling, evaluation and assessment
of needs. The CRT Case Manager shall maintain a close working
relationship with all members of the treatment team in order to ensure
delivery to clients of comprehensive treatment in an efficient and
expedient manner.
Responsibilities:
Participate and lead, as directed, in the screening of new service
requests within the clinic through means of and agency approved
procedures which ensures that all individuals are adequately and
appropriately served according to their individual needs and in
accordance with existing agency policies and procedures.
Accept newly assigned cases in which the diagnostic intake has been
completed by a licensed provider. Complete case management assessment
and develop an individualized treatment plan with the client based on
that assessment for all clients on caseload. Provide on-going supportive
and/or case management functions in accordance with the problems, needs,
and the strategies identified within the treatment plan in order to help
the clients to achieve the stated goals and objectives.
Provide on-going supportive and/or mandatory monthly case management
contact, activity or communication in accordance with the problems,
needs, and the strategies identified within the treatment plan in order
to help the clients to achieve the stated goals and objectives.
Document all service contacts on a timely basis including face-to-face
interviews, collateral and networking contacts, and/or correspondence in
addition to maintaining case records in accordance with agency and
regulatory standards and requirements.
Qualifications:
Associates degree in psychology, human services, social work or related
field.
Possess 1-2 years combination of mental health work experience and
training to perform principle responsibilities.
Knowledge with services, systems, and programs available in the
community including primary health care, support services, eligibility
criteria and intake processes, generic community resources,
Working knowledge of serious mental illness, mental disabilities, and
substance use depending on the population served, including clinical and
developmental issues.

with personal automobile liability insurance coverage at or above the
levels mandated by the State of Vermont.
*Successful completion of a criminal history background check after
hire.
