Conviva is a Subsidiary of Humana, seeking a dedicated LPN/LVN, in this role you will be a part of the Clinical Care Team engaging with our patients to develop lifelong well-being and health. The LPN/LVN role makes a difference in the patient care we offer and assists physicians and other clinicians by providing a unique blend of skills and knowledge to carry out prescribed medical treatment. LPN/LVN are multiskilled health professionals responsible for performing administrative and clinical tasks in our primary care facilities while delivering outstanding customer service and maintaining positive patient engagement. Job Functions

  • The LPN/LVN assesses patient health problems and needs.

  • Takes vital signs, administers medication, develops and implements nursing care plans, and maintains medical records.

  • May advise patients on health maintenance and disease prevention or provide case management.

  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences.

Required Qualifications

  • Current licensure as an LPN or LVN

  • Bilingual (English, Spanish)

  • A minimum of one year of LPN/LVN experience in a clinic or hospital setting.

  • IV Certified/Willing to learn.

  • Must be able to work at the Conviva Care clinic located at:

  • 12602 Toepperwein #100 San Antonio,Tx 78233

Preferred Qualifications

  • Hands on professional- Phlebotomy experience

  • Experience with Electronic Medical Records, specifically eCW (eClinical Works)

  • Experience with HEDIS

  • Knowledge of primary care medicine requirements (state specific)

Working hours : Monday to Friday 8AM-5PM

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.$44,200 - $60,900 per year
