Job Purpose:
Duties and Responsibilities:
Provide culturally sensitive counseling and rehabilitation services
Facilitate referrals to community resources and perform case management
Conduct psychosocial assessments and formulate treatment plans
Document and maintain clinical records per standards and regulations
Participate in clinical supervisions, team meetings, and trainings
Collaborate with the primary care team to integrate behavioral health into patient care plans
Complete and maintain LPS 5150 Authorization certification and provide crisis intervention support
Other related duties and projects as assigned
Qualifications:
Ability to perform duties described above
Knowledge of laws and ethical standards in clinical care
Proficiency in electronic health records and documentation
Verbal and written fluency in English required; bilingual skills desired

Pay Details: $71,000.00 to $130,000.00 per year
