Location: 56 Bay Street, 4th Floor Staten Island, NY 10301

Position Status: Part-Time (24 hrs / week)

FLSA Status:Non-Exempt

Community Health Action of Staten Island (CHASI)drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI serves the most vulnerable individuals, families, and communities with critical services and programs. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – the poor and working poor, low income people with chronic illnesses, persons with criminal justice involvement, substance users, domestic violence survivors, people of color, persons living with HIV/AIDS, and the LGBTQ+ community.

POSITION SUMMARY:The Engagement and Support Services (ESS) Program assists People Living with HIV to improve overall health and establish self-management through case management, peer navigation, and health education services with a focus on improving medical outcomes, achieving viral suppression, and enhancing self-management skills. The Community Health Worker (CHW) position was created using the research and belief that involving people who have their own lived experience with substance use, mental health conditions, and/ or HIV is invaluable in reaching and helping the population. The CHW mentors and guides individuals using their experiences to support people in a way that shows people that living a healthy life with HIV is not only possible but sustainable. The CHW maintains appropriate positive, professional interpersonal relationships with staff, participants, volunteers, and the community. The CHW will be well versed in all CHASI services to refer and link clients appropriate to services. The CHW’s responsibilities will align all activities with CHASI’s mission, core values, and culture.

DUTIES & RESPONSIBILITIES:

  • Provide an overview of program, department, and agency services through Orientation activities.

  • Accompany participants to provider sites for support and navigation assistance.

  • Assist with community outreach and program promotion to inform the public of available agency resources.

  • Assist the Case Manager and Health Educator in the screening, assessment, and monitoring process.

  • Provide reminder phone calls to reinforce retention in care and services.

  • Reach out to participants to follow up on referrals made and to engage them in programming.

  • Maintain regular contact with participants to identify and address barriers to care and treatment.

  • Outreach to individuals who miss appointments to minimize lapses in care.

  • Recruit clients for Case Management and Health Education through internal and external outreach.

  • Provide screenings and determine eligibility for those interested in services.

  • Co-facilitate the Health Education Group; provide information about maintaining and increasing health and wellness.

  • Attend and participate in staff development training.

  • Actively participate in regularly scheduled supervision and attend team meetings/daily huddles.

  • Complete all program documentation in a timely manner.

  • Other duties as assigned.

Requirements

QUALIFICATIONS:

  • The Community Health Worker will be an NYS AIDS Institute Certified Peer, preferred. Peer certification from a reputable program can also be considered.

  • Candidates with lived experience with substance use, mental health conditions are welcomed, howeverlived experience with HIV is highly preferred.

  • High school diploma or high school equivalency is highly preferred.

  • Bilingual Spanish is highly desired.

  • Possess an understanding of community-level work and the importance of collaborating and coordinating with other organizations.

  • Effective oral and written communication skills.

  • Ability to juggle priorities and manage time productively; thrives in a fast-paced environment.

  • Knowledge of community resources and organizations on Staten Island.

  • Demonstrate organizational & problem-solving skills.

  • Proficient in Microsoft Word, PowerPoint, and Excel.

  • Familiarity with electronic health records or a willingness to learn.
