Pay Rate: $23-$26.50 per hour
Pay within this range is based on experience and education
Duties and Responsibilities
Administer medications to residents in accordance with the Residential Medication policies and procedures
Accurately and consistently complete all required documentation practices
Complete all necessary follow-up activities for "secondary mark" incident reports
Attend resident’s medical/health related appointments and other meetings as requested
In conjunction with the Residential RN and the Med Liaison provide orientation of new AMAP staff to the med room and med training activities
Assist the Residential RN, as directed, in medication administration practicums and follow-up observations
Assist in the personal and physical care needed by each individual as needed (including tasks such as bathing, toileting, eating and dressing)
Perform housekeeping tasks as designated by the shift supervisor/Residence Coordinator (including tasks such as meal preparation, cleaning and laundry) if needed
Implement fire, safety and emergency procedures and assist with fire drills
Assist in transportation to and from medical appointments as required
Maintain Agency-Approved Driver status and complete all required ongoing/annual trainings and in-services, as assigned
All other duties as assigned
Minimum Qualifications
Licensed Practical Nursing licensure
6 months or more experience working with individuals with developmental disabilities
Ability to read, write, speak and understand English
Possess a current and valid NYS driver’s license with an acceptable driving record
Be able to regularly lift and/or move up to 50 pounds
Preferred Qualifications
- Experience in an OPWDD certified residential setting
Certifications and Trainings
Must maintain LPN Licensure
NYS Medication Administration
CPR
First Aide
SCIP-r
All other trainings mandated by the OPWDD
