Forgiveness (PSLF) program GENERAL FUNCTION Assess, plan and coordinate
needed support and services to children who have physical, cognitive,
and emotional disabilities. Service Coordinators also support the
immediate family members of these children and help problem solve
issues/concerns that arise. This position has a significant impact on
the children and families served, as well as on the organization, as it
allows Wood County children to receive the items and services they are
in need of, to keep them safe and allow them to stay within their home
and community, (least restrictive and most natural environment) rather
than having them placed in more restrictive and less natural
environments. RESPONSIBILITIES ESSENTIAL FUNCTIONS 1. Conduct
assessments and develop and implement Individual Service Plans for
children in the Children\'s Community Options Program (CCOP) and the
Children\'s Long-Term Support (CLTS) Medicaid Waivers Program. 2.
Provide direct supportive services to children and families on caseload
including information/referral, supportive counseling, assistance in
making application for financial assistance programs, etc. 3. Conduct
home visits and/or set up virtual video visits to discuss family
issues/concerns and document all relevant information within CCOP and
CLTS Medicaid Waivers Program. 4. Complete Functional Screens (initial,
annual, updates). Update Children\'s Functional Screens annually. 5.
Complete paperwork other than Functional Screens (case notes, letters,
emails, faxes, funding requests/authorizations, WPS authorizations,
Individual Service Plans (ISP), Outcomes, referrals, reading over
medical records, reviewing Individual Education Plans (IEP), reviewing
parents/guardians requests of items/services, completing incident
reports through Citr, entering data into the ForwardHealth Portal using
Eligibility and Enrollment Streamlining (EES), high cost funding
requests, parental fee calculations, other state and county forms,
assessments, etc.). 6. Make home visits to conduct intakes, reviews,
check-ins, provide support, attend school meetings and/or medical
appointments, etc. 7. Make telephone calls to clients, providers, state
contacts, therapists, evaluators, contractors, and other professionals
and non-professionals associated with the provisions of the County
operated services. 8. Create and help manage CLTS Waiver Third Party
Administration TPA authorizations with the Business office staff. 9.
Perform all duties in compliance with the Medicaid Home and
Community-Based Services (HCBS) Waiver Manuel for the CLTS Waiver
Program and standards set out in Medicaid Targeted Case Management

list of duties. ***** OTHER EX ERIENCE AND QUALIFICATIONS: -Must be
knowledgeable of the service delivery system, the availability of
integrated services and resources, or the need for such services and
resources to be developed. -Must complete the approved DHS waiver basics
training course and the Mandated Reporter Online Training. Until
training is completed, the Coordinator must work under the direct
supervision of a qualified Support Service Coordinator and/or
supervisor. -Candidates must possess basic everyday living skills, the
ability to understand and follow directions, the ability to read and
write (for reports); and the ability to interview, counsel and
communicate. This position requires travel to and from off-campus sites.
-Possession of a valid driver\'s license, as well as a licensed and
insured automobile is required, or other means of reliable
transportation immediately available to individual. Basic office
equipment used: computer/tablet, printers, telephone, copy machine, fax
machine and calculator is required, as is operational familiarity with
Microsoft Word and Excel software and Streaming Healthcare Solutions
