We are currently hiring a Group Visit Nurse Manager based in Worcester. The Group Visit Nurse Manager Coordinates group visits for Kennedy Community Health patients. Fosters a team-based relationship with all members of the medical and behavioral health departments, continually working towards integration into Primary Care.

Essential Functions:

  • Manages the day-to-day operations of the in-person and/or hybrid group programs Monday through Friday including scheduling, staffing and leadership of group visits.

  • Ensure Group visits schedules are appropriately filled, and no-show patients are re-engaged.

  • Be able to assist Group Visit programs as needed in person.

  • Provides a wide spectrum of services as part of a primary care team, linking patients to appropriate services that promote positive health outcomes, treatment adherence, and self-sufficiency.

  • In collaboration with Clinical Program Nurse Manager, support ongoing grants and compliance with grant requirement and reporting.

  • Develop organizational workflows and processes.

  • Supervise any future staff.

  • Ensure that appropriate clients are referred internally and within the community and provide follow-up.

  • Foster collaborations between internal and external healthcare teams to ensure the delivery and implementation of an integrated model of care.

  • Works in close collaboration with the Office Based Addiction Treatment (OBAT), Gender Affirming Care (GAC) and Reproductive Health teams.

  • Through regular chart reviews, ensures that all services are appropriately documented in patients’ electronic health records.

    Education and Experience:

  • Unrestricted and current MA Nursing License

  • Current Basic Life Support (BLS) certificate.

  • 2 years of external experience. 1.5 years of experience at Kennedy Community Health.

  • Our health center requires all employees to have the most recent COVID-19 booster and the yearly flu vaccine.
